Act as the primary point of coordination for individuals with developmental disabilities. Assess individual needs for services and develop individual service plans with the active participation of the individual. Establish and implement an ongoing system of monitoring the implementation of individual service plans to achieve the desired outcomes for the individual. Ensure that services are effectively coordinated and provided by appropriate providers.
Bachelor’s Degree in psychology, social work, or other closely related field according to DODD standards. Preferred to have a minimum of two (2) years of experience working directly with persons with DD. Must have or be eligible to obtain Service and Support Administration certification from DODD. The applicant shall pass background checks, including but not limited to BCII. Must maintain valid Ohio Driver’s License and proof of insurance.
