We believe everyone deserves to live their life in the greatest comfort and dignity possible. With the help of our community’s residents, we are able to fulfill our duty to provide a comprehensive range of quality homecare, hospice and palliative care services, designed to assist our patient’s needs. These services will assist our patients in achieving and maintaining their optimal level of independence and highest quality of life.
Homecare: In general, the goal of home health care is to treat an illness or injury. Where possible, home health care helps you get better, regain your independence, and become as self-sufficient as possible. Home health care may also help you maintain your current condition or level of function.
Hospice: The philosophy of Putnam County Hospice is the belief that dying is an integral part of living. Hospice allows one to live each day fully and as alert and pain-free as possible. Hospice treats the person along with the disease. We encourage the celebration of life every day.
Emotional, physical, spiritual and social support is provided to the patient and family throughout the process of both illness and bereavement. Family members are encouraged to be actively involved in their loved one’s care. A team of physicians, nurses, social workers, clergy, aides and volunteers follows an individualized care plan designed to meet the specific needs of the patient. In addition to regular nursing visits, a nurse is on call 24 hours per day to assist the families as needed.
Palliative Care: Palliative care is defined as “relieving or soothing the symptoms of a disease or disorder.” Many people mistakenly believe this means you receive palliative care only when you can’t be cured. Actually, palliative medicine can be provided by one doctor while other doctors work with you to try to cure illness.
