Eligibility Referral Specialist II

Summary

The Putnam County Department of Job and Family Services is seeking a dynamic and dedicated Eligibility Referral Specialist II to join our team.

POSITION OVERVIEW:

· Determine eligibility for Prevention, Retention and Contingency (PRC) program, Medicaid, and Long- Term Care.

· Conducts phone interviews and accesses the Ohio Benefits Worker Portal to perform case management to determine eligibility for assistance for Temporary Assistance for Needy Families (TANF) and Supplemental Nutrition Assistance Program (SNAP)

· Processes change that affect eligibility for assistance.

· Processes applications on a daily basis by completing new applications and reapplications for assistance and mailing out applications as needed

· Completes referrals for the Able-Bodied Adults Without Dependents (ABAWD) program,

· Contact clients to answer questions and provide updates to cases,

· Scans all client paperwork using the electronic documents management system (EDMS) and tracks all due dates of follow-up items.

· Sends all needed paperwork to clients and updates all cases for increases or decreases of assistance.

· Sends requests for verification for eligibility

Experience

MINIMUM QUALIFICATIONS:

High School Diploma or GED. Associate degree preferred. Valid driver’s license. Previous case management experience preferred. Strong written and verbal communication skills, be skillful in problem solving, maintain accurate records and be able to develop and present new ideas to unique issues as they arise. Previous experience with Microsoft Office preferred. You must be highly adaptable to change, organized, ability to meet multiple deadlines, and want to make a difference in the lives of families in the community. Previous experience with case management and social services programs preferred.

PAY RANGE:

$39,000 to $49,774.40 annually
