The Putnam County Council on Aging (PCCOA) is looking for an energetic, self- motivated, and compassionate professional with expertise in social work or related field to join our team.  This is a full-time position with benefits.  The successful candidate must have strong interpersonal communication and organizational skills.  The Social Worker shall be responsible for assessing the needs of older adults and connecting them to services.  This may include assistance in completing applications, applying for services, providing assistance to resolve client concerns and information and referral services.  The Social Worker is also responsible for facilitating evidenced-based programs such as fall prevention classes along with building relationships with community partners, attending community meetings and health fairs, serving on committees, and providing public speaking engagements.  The social worker is also responsible for maintaining accurate, confidential and up-to-date client records and outreach reports.  The Social Worker shall cooperate with co-workers to function as a team to accomplish the agency’s mission of providing services to Putnam County residents, age 60 and better, to help them remain independent in their own home.
